چکیده

Chronic heart failure is a major and increasing cause of morbidity and mortality. Although it is widely recognized that chronic heart failure affects the elderly, little attention has been given to its study specifically in this patient group. For example, patients with chronic heart failure in clinical trials frequently have an average age of around 60 years, in comparison to one of nearer 80 years in epidemiological studies. The importance of examining the specific characteristics of chronic heart failure in older individuals is underlined by predictions of an ‘epidemic’ of chronic heart failure that will largely be driven by the combination of an ageing population (by 2030 it is projected that there will be 70 million adults in the United States alone greater than 65 years) and improved survival following acute myocardial infarction. There is already evidence that the age of patients with chronic heart failure is increasing over time.

Protease corin expression and activity in failing hearts.

Atrial and brain natriuretic peptides (ANP and BNP) regulate blood pressure and cardiac function. In patients with heart failure (HF), plasma levels of pro-ANP and pro-BNP, the precursor forms of ANP and BNP, are highly elevated, but the mechanism underlying the apparent deficiency in natriuretic peptide processing is unclear. Corin is a cardiac protease that activates natriuretic peptides. In ...

Transmural heterogeneity and remodeling of ventricular excitation-contraction coupling in human heart failure.

BACKGROUND Excitation-contraction (EC) coupling is altered in end-stage heart failure. However, spatial heterogeneity of this remodeling has not been established at the tissue level in failing human heart. The objective of this article was to study functional remodeling of excitation-contraction coupling and calcium handling in failing and nonfailing human hearts. METHODS AND RESULTS We simul...

Myocardial dysfunction in donor hearts. A possible etiology.

BACKGROUND Potential cardiac donors show various degrees of myocardial dysfunction, and the most severely affected hearts are unsuitable for transplantation. The cause of this acute heart failure is poorly understood. We investigated whether alterations in calcium-handling proteins, beta-adrenoceptor density, or the inhibitory G protein Gialpha could account for this phenomenon in unused donor ...

Effects of morphine on atrial preparations obtained from nonfailing and failing human hearts.

We have examined the effects of morphine in auricular myocardium from non-failing and failing human hearts. In both preparations morphine induced inhibition. These responses were not antagonized by naloxone. Comparison of mean IC30 values obtained in non-failing (3.9 (SEM 0.2) x 10(-8) mol litre-1) and failing (5010 (200) x 10(-8) mol litre-1) hearts indicated that the morphine concentration-re...

Left Ventricular Assist Device Effects on Metabolic Substrates in the Failing Heart

BACKGROUND Heart failure patients have inadequate nutritional intake and alterations in metabolism contributing to an overall energy depleted state. Left ventricular assist device (LVAD) support is a common and successful intervention in patients with end-stage heart failure. LVAD support leads to alterations in cardiac output, functional status, neurohormonal activity and transcriptional profi...
